Bridging Access to Care
Bridging Access to Care (formerly known as Brooklyn AIDS Task Force) was the first HIV/AIDS organization in Brooklyn. Their mission is to provide health care and supportive services to help individuals living with HIV, living with mental health conditi0ns, substance users, seeking healthier lifestyles, and healthier families.

Callen-Lorde Community Health Center​
Callen-Lorde Community Health Center provides quality health care and related services targeted to New York's lesbian, gay, bisexual, and transgender communities, regardless of ability to pay. To further this mission, Callen-Lorde promotes health education as well as wellness and advocates for gay, lesbian, bisexual, and transgender health issues.

Community Counseling and Mediation
Community Counseling and Mediation (CCM) works to improve the lives of at-risk children and families through innovative behavioral health, supportive housing, education, and youth services. Their expanding network of licensed mental health and substance clinics abuse, award-winning housing facilities, HIV and hepatitis prevention and care coordination, and after-school and child-welfare programs, utilize evidence-based, culturally competent practices to achieve positive outcomes and keep families together.

Community Health Action of Staten Island (CHASI)
CHASI provides extensive health services for individuals and groups, including free HIV and hepatitis C testing, workshops for managing chronic illness, and safer sex education. They will also connect you to any other health services you may need. They offer benefits counseling daily and HIV testing.

Health Outreach To Teens (HOTT)
Health Outreach To Teens (HOTT) is a welcoming, non-judgmental, confidential program designed specifically to meet the medical and mental health needs of lesbian, gay, bisexual, transgender, and questioning adolescents and young adults ages 13-24, as well as homeless youth, runaways, sex workers, squatters, and other street-oriented youth. These services are offered both at a youth-only medical suite at Callen-Lorde, and on HOTT's medical van, which travels to areas throughout Manhattan in the afternoons and evenings where youth are known to hang out. All of HOTT's services are provided free of charge or at low cost (insurance is also accepted).

Hispanic AIDS Forum
​The Hispanic AIDS Forum's (HAF) mission is to improve health outcomes for Latinos in New York City who are affected by HIV and other chronic illnesses. They offer free HIV testing; emotional support counseling and support groups or gay and bisexual Latino men; services for HIV-positive Latinos, such as help managing medical care and treatment, financial assistance with rent payments, plus health education and wellness services; Latino youth leadership training; and services for transgender women.

Housing Works Community Healthcare
Housing Works Community Healthcare fosters good health and positive social change through empowerment, innovation, and collaboration. They provide high-quality integrated medical care and other essentials that improve individual and overall community well-being. Latino Commission on AIDS
The Latino Commission on AIDS is a nonprofit membership organization dedicated to fighting the spread of HIV/AIDS in the Latino community. The Commission's public health model encompasses four core and complementary services provided to Latino communities: health education, HIV prevention, capacity building, and advocacy. All services are offered in Spanish by a culturally diverse bilingual staff of health, education, and business professionals. ​

The Oval Center at Montefiore
​Provides state-of-the art prevention, screening, treatment, and sexual health services in a discreet, comfortable, and convenient setting. Services include: confidential screening and treatment for sexually transmitted infections (STIs); rapid HIV testing; pre-exposure prophylaxis (PrEP) for individuals at high risk for acquiring HIV; post-exposure prophylaxis (PEP) for individuals who have had recent high risk sexual encounters; vaccinations, including hepatitis B and HPV; sexual healthcare for LGBTQ individuals; hepatitis C screening and treatment; sliding fee scale and assistance obtaining insurance or those in need.

RAD Remedy
RAD Remedy's mission is to connect trans, gender non-conforming, intersex, and queer folks to accurate, safe, respectful, and comprehensive care in order to improve individual and community health. RAD is a comprehensive and nationally-collaborative database of providers based on referral lists of trusted community organizations on detailed reviews of community members.  ​

Services For The UnderServed (S:US)
Services for the UnderServed (SUS) offers continuum of care to those recovering from substance abuse, co-occurring disorders, trauma, etc. Outpatient and residential programs provide domestic violence treatment, HIV/AIDS services, primary healthcare, alternative criminal justice rehabilitation, vocational training and placement, mental health treatment, and supportive permanent housing services.
